Blue Goji Parental Notice and Consent
Attention Parent/Legal Guardian:
Someone is attempting to create a user account for a Blue Goji LLC (“Blue Goji”) product or service, such as an Expresso bike or Goji Play app, for your child under the age of 18. In order for your child under the age of 18 to have a user account for a Blue Goji product or service, we are required to obtain your permission to our online collection and use of your child's personal information.
With your permission, we may collect certain personal information from your child, such as his or her e-mail address, demographic and certain biometric information, precise location information (in connection with some of our apps), information about your child’s activities and use of Blue Goji equipment, apps, games, challenges, participation in our online community, identification on leaderboards and similar public sharing of activities and other information as described in full in our Privacy Policy (www.bluegoji.com/privacy).
At any time, you may withdraw your consent to our collection of personal information online from your child by giving us written notice at privacy@bluegoji.com. Upon receipt of your request for withdrawal of permission for your child, we will disable your child’s user account.
Once you have reviewed our site and our Privacy Policy, please fill out and sign the attached consent form (using a separate form for each child for whom you grant permission), and return the signed consent to us by either: (i) email of the scanned consent to privacy@bluegoji.com, or (iii) mail to us at: Blue Goji LLC, attn: Legal Dept, 6301 E Stassney Lane, Bldg 6 Suite 400, Austin, TX 78744.
If you do not provide your consent or do not respond within ten (10) business days, we will not collect personal information from your child online nor enable the creation of a user account for your child, and we will delete your contact information.
